Henna Service Consent & Intake Form (Client Statement) 

I understand that henna is a natural plant-based dye, and while generally safe, it may cause allergic reactions or skin sensitivity in rare cases. I confirm that I do not have any known allergies to natural henna, essential oils, lemon juice, or other ingredients typically used in the henna paste. I have not experienced any previous reactions such as rashes, itching, burning, or discoloration due to henna or similar body art products. If I have a history of sensitive skin, eczema, or allergies, I acknowledge that I have disclosed this information to the artist in advance. I understand that black henna is not used due to the risk of serious allergic reactions. I consent to receiving the henna service at my own risk and release the artist from any liability related to allergic or skin responses. I agree to follow the aftercare instructions provided for the best stain results and skin safety.
